Duplicated Chat Rooms & Missing Organization Members in Space Chat

We're encountering two critical issues that are directly impacting the usability of the platform for ISTITUTO FORMATIVO ALADIA:

1. Duplicated Chat Rooms
   Chat rooms are being duplicated upon creation or access. This causes confusion among users and disrupts communication workflows.

2. Organization Members Not Added to Space Chat Room
   Organization members are inconsistently added to space chat rooms. Sometimes users are added as expected, but in other instances, members are missing, which fragments conversations and isolates participants.
