Attachments not updating correctly

When editing a lecture in the Course Builder:

1. Add an attachment to the lecture.

2. Remove the attachment.

3. Add a different attachment.

4. Save the lecture.

Issue:
Both the first (removed) and second attachment are saved, instead of only saving the second (current) attachment.
